My 2-fold method is so unique because it covers the HUMAN ELEMENT of the recipients of your exemptions, along with the standard LEGAL REQUIREMENTS for submission. Combine that with your unique personal story, interweaving your religious beliefs, it’s guaranteed that will separate you from all the “copy and paste” people who get denied using reckless templates from the Internet!
The biggest misconception is people think religious beliefs must be organized religion. NO!
According to the EEOC: “The law protects not only people who belong to traditional, organized religions, such as Buddhism, Christianity, Hinduism, Islam, and Judaism but also others who have sincerely held religious, ethical or moral beliefs.” So there is a CLEAR distinction between organized religious beliefs and “others who have sincerely held RELIGIOUS, ethical or moral beliefs”
And the Canadian Supreme Court determined, “an individual advancing an issue premised upon a freedom of religion claim must show the court that he or she has a practice or belief, having a nexus with religion, which calls for a particular line of conduct, either by being objectively or subjectively obligatory or customary, or by, in general, subjectively engendering a personal connection with the divine or with the subject or object of an individual’s spiritual faith, irrespective of whether a particular practice or belief is required by official religious dogma or is in conformity with the position of religious officials [Syndicat Northcrest v. Amselem (2004)]
You’ve had your religious beliefs for as long as you were able to tell right from wrong, answering to your God-given gift of conscience so the TRUTH SHALL SET YOU FREE!

What is the specific sincerely held religious belief, practice or observance for which you are seeking accommodation?
Please describe the conflict between your religious belief or practice and the Covid-19 vaccination workplace requirement?
Please describe whether, as an adult you have received any vaccines against any other diseases (such as a chicken pox, influenza, measles, tetanus, etc) and, if so, what vaccine did you most recently received and when, to the best of your recollection?￼
Does your religious belief require you to abstain from all vaccination or just covid-19?
When did you first develop the religious beliefs that prevent you from receiving the Covid-19 vaccine?
[If the workplace requires a regular flu vaccine] why have you never requested a religious-based exemption from the flu vaccine?
If you do not have a religious objection to the use of all vaccines, please explain why your objection is limited to particular vaccines?
If your religious beliefs prevent you from receiving other vaccinations, is this information available in the medical care record that can be provided upon request?
If there are any other medicines or products that you do not use because of the religious belief underlying your objection, please identify them?
Have you ever taken any other prescription or non-prescription drugs to prevent an illness/condition?
How do your religious beliefs distinguish between the covid-19 vaccine and other procedures you have undergone, and prescription and nonprescription drugs you have taken since the age of 18?
Get help with the RELIGIOUS LEADER ATTESTATION “requirement”:
For USA: According to federal law, 29 CFR § 1605.1, “The fact that no religious group espouses such beliefs or the fact that the religious group to which the individual professes to belong may not accept such belief will not determine whether the belief is a religious belief of the employee or prospective employee.” Therefore, any organized religion or denomination (and their religious leader) does not have to espouse your belief for it to be sincerely held so you cannot be denied your right to accommodation for not having a religous leader sign off on your beliefs.
For Canada: According to Little v. R., (2009) P7 , the courts held, “a protected religious practice need not be part of an established belief system or even a belief shared by others. An individual need only demonstrate a sincere belief that the practice is of religious significance to him or her.” Therefore, a protected religious practice does not need to be a belief shared by others, including religious leaders.
